Licensed Medical Social Worker (LMSW / LCSW)
Home Health & Hospice | Full-Time | New Mexico

Guardian Angel Home Health is a well-established, mission-driven agency providing Home Health, Hospice, and Personal Care Services across New Mexico. We are growing and looking for a Licensed Medical Social Worker who wants meaningful, relationship-based work with patients and families in their homes, not rushed, checkbox care.

What You’ll Do

• Provide compassionate social work services to home health and hospice patients and their families

• Support patients through psychosocial challenges, care planning, and end-of-life transitions

• Collaborate closely with nurses, therapists, physicians, and the hospice team

• Connect patients and families with community resources and support services

• Advocate for patient needs while honoring hospice philosophy and patient-centered care

What We’re Looking For

• Master’s degree in Social Work

• Current New Mexico social work license (LMSW or LCSW)

• At least 1 year of healthcare or hospice experience (hospice experience strongly preferred)

• Strong understanding of end-of-life care, grief, and family systems

• Knowledge of local community resources

• Valid New Mexico driver’s license and reliable, insured transportation
